Iron levels are low and weight is identical to the previous visit last week. If LG doesn’t gain more weight (preferably 10 pounds) before the birth of the baby the baby can be born prematurely and underweight. Other risks for babies are breathing problems (respiratory distress syndrome), an increased risk of infection, and hypoglycaemia. Interventions Diet and weight gain monitoring Weekly visits Education Expected outcomes LG understands lessons from today’s education LG gains the necessary weight before the birth Next week, LG’s levels will be improved We will have a healthy baby Goal of care/appropriate diet *LG requires a dietary modification of her regular diet with no external feedings by tube necessary* Eat more frequently (5 or 6 small meals every day) Choose nutrient and calorically dense foods (nuts, dried fruits, ice cream, and lean meats) Add some fats to meals by using condiments such as sour cream, cheese, or margarine) Drink 8 glasses of water daily Protein and iron supplements are to be consumed qd (every day) Nutrition education I will teach LG how to establish good fats from empty fats to ensure adequate weight gain. She will also be taught how to properly count calories since increasing daily caloric intake by 300 calories qd is necessary. Aside from food, LG will be taught what nutrients and supplements she need to increase to reduce the change of having a baby with birth defects. Some important ones are folic acid, iron, and calcium. To avoid potential herb/nutrient/ drug interactions, LG should consult a physician or pharmacist before taking anything due to risks of harming the baby. Specific food or beverages including dairy products can decrease the absorption of iron from the stomach (importance of iron supplements).
