The student will write a scholarly paper that analyzes the DNP project implementation process, including management, leadership, participants, staff, and access to data/information technology. This assignment is a continuation of Assignment #4: Mid-Semester Project Implementation Process Paper from mid-semester to the end of the project and semester. This is the beginning of Chapter 4 of the DNP Project Report.
Assignment Criteria:
1. Discuss the recruitment, consenting, and intervention implementation.
2. Discuss the sample participants
3. Discuss the factors that are facilitating or inhibiting the project implementation:
a) Project Management: Any EBP or QI process used during the implementation
b) Access to data and information technology
4. There is no page limit to this assignment, as only the student knows the length, they must go to in order to provide a logical and cohesive argument as to why their choice of project is a current clinical problem/issue.
5. The scholarly paper should be in narrative format and include a title and reference page(s).
6. Include an introductory paragraph, purpose statement, and a conclusion.
7. Include level 1 and 2 headings to organize the paper.
8. Write the paper in third person, not first person (meaning do not use ‘we’ or ‘I’) and in a scholarly manner. To clarify: I, we, you, me, our may not be used. In addition, describing yourself as the researcher or the author should not be used.
9. Include a minimum of 5 professional peer-reviewed scholarly journal references to support the paper (review in Ulrich Periodical Directory) and be less than five years old.

This agreement should be kept at the office where the psychiatric mental health nurse practitioner (PMHNP) is working and available for inspection. This assignment provides the student with an opportunity to design a collaborative agreement for a psychiatric mental health nurse practitioner (PMHNP) position.

For this assignment, students will create two SOAP notes reflective of the patient care experience in the clinical setting under the supervision of the clinical preceptor in the role of the clinical provider. This assignment will evaluate the student clinical reasoning skills, interviewing skills, physical exam skills, selection of diagnostic testing, differential diagnosis, pharmaceutical and non-pharmaceutical treatment, patient education, and follow-up plan.

Students must develop the clinical skills and knowledge required for safe practice and deliver best patient outcomes upon graduation. SOAP notes should be used to document each patient seen in the clinical setting. Clear, concise, and thorough documentation is required for continuity of care, safe practice, appropriate reimbursement, and prudent risk management.

When developing the SOAP note, students should use the assignment criteria below and the ACON SOAP Note Template found in Modules. Students should include complete subjective and objective information to support the assessment and plan. The plan must include diagnostic and treatment measures, patient education, and follow-up.

4. Plan
a. Identify and orders correct diagnostics, prescriptions, referrals, and follow-up plan

c. Correctly written out a prescription for one medication prescribed for the patient.
i. If a medication not prescribed, write out a prescription for a medication that might be prescribed for a similar patient
5. Include two current evidence-based guidelines and/or peer-reviewed scholarly journals to support patient education and treatment plan.
