Hello Colleagues:

My name is Karen Matthew, and I am a BSN to DNP program student. I started this journey at Nova in 2021 and will complete the program at the end of this semester. I have been a nurse since 2014 and I have worked in multiple environments including inpatient pediatric oncology, general pediatrics, quality improvement, outpatient clinics, and administrative roles. This has made for a challenging yet interesting nursing experience. After completing this program, I would be interested in working with outpatient care management, research and process implementation, and teaching and mentorship. This may sound like a lot, but I believe it can be achieved in the right environment. The limited spare time that I now have is spent keeping my soon-to-be 12-year-old son entertained by involving him in various sports, and musical instruments, and supporting him on his education journey.

This semester I aim to complete Project IV and the Health Policy class, and of course, complete the board exam. My DNP project is about cholesterol education in the cystic fibrosis population. Cystic fibrosis is a genetic disease that is thought to affect about 100,000 people globally (Shteinberg, et al., 2021). Recent innovations in disease management have allowed for many positive changes in patient outcomes and have also increased the potential for the development of hypercholesterolemia in these patients.

I used an initial cholesterol knowledge questionnaire to assess baseline knowledge, and then a website as an educational intervention. After interacting with the website, participants again completed the questionnaire. During analysis, in addition to collecting some baseline and demographic data, improvement in the questionnaire responses will also be assessed. I used REDCap so that the entire process was electronic. The main goal of this was to allow for some flexibility with patients, allowing them the opportunity to complete the activity after the appointment at their convenience. Most participants were excited about that option, but challenges arising as a result of this included people failing to complete the project once they were no longer in the controlled environment, and a smaller number of people who were more inclined to paper rather than electronic interactions and so elected not to participate.

Another consideration I had to make was to accept my lower-than-expected participation rate. Initially, I had expected upwards of 50 participants. There were some issues, like the amount of time allowed for recruitment, and the frequency of clinics (one day weekly), the clientele during the clinics (not all patients at the clinic met the inclusion criteria), and participant follow-through to actually complete the project. At this time I have around 10 completed surveys. I will leave it open for a little while longer, but I don’t anticipate a significant increase.

This has been a long time coming, and we can finally see the light. I look forward to sharing information with you all this semester and to learning more about your projects. Best wishes as we continue to head for the finish line.

Introduction

Hello. My name is Kyla, and I am a BSN to DNP PMHNP student. I moved to Florida in 2021 and currently work as a psychiatric nurse on an acute inpatient unit. In addition to this course, I will also complete Project IV this semester. Psychiatric medicine has always been my passion, and I am grateful to continue my education in an often-overlooked field. I look forward to continuing my learning alongside my classmates in the coming months!

Nursing Engagement in Policy

The importance of policy development has grown increasingly apparent as a crucial component for advancing healthcare delivery and practice. Policies can be defined as the rules, laws, or regulations that define the way individuals live in a society (O’Rourke & Outly, 2024). Policies exist at many levels; however, in the broader context, public policy is often shaped by the government and specific key stakeholders (Disch, 2020; O’Rourke & Outly, 2024). Policy development follows a cyclical process with four phases. These phases include agenda setting, policy development, implementation, and evaluation (Scott & Scott, 2021). In the healthcare sector, this applies to health policy that outlines acceptable standards of care and allocates resources to ensure conditions are conducive to high-quality patient care (Scott & Scott, 2021).

Currently, nurses have the capacity to engage in advocacy and policy change at the legislative level, both independently and as members of professional organizations (O’Rourke & Outly, 2024). This engagement is not just a formality, but a powerful tool that can shape the future of healthcare. Nurses can leverage the power of their professional organization to write letters to members of Congress, testify before state legislatures, and build working relationships with legislators to facilitate policy objectives. Additionally, some nurses can serve as state or national legislators, while a select few may serve in local, state, or federal governmental positions (Disch, 2020). Despite available opportunities, Scott and Scott (2021) assert that nursing engagement in policy and advocacy is low. Survey results indicated that across 1,000 hospital boards in the U.S., 20% of board members were doctors, while only 6% were registered nurses (Disch, 2020). Additionally, 50% fewer nurses were serving in the U.S. House of Representatives in 2023 versus 2015, while nurses in state legislatures have declined by 30% since 2022 (O’Rourke & Outly, 2024). This has resulted in 16 states having inadequate nursing representation during meetings regarding health policies.

Nurses’ Hesitation in Policy

Researchers argue that nurses are hesitant to engage in policy development and advocacy due to a myriad of barriers. Scott and Scott (2021) argue that increased workloads, inadequate staffing, time insufficiency, and feelings of powerlessness likely contribute to reduced participation. Another commonly cited inhibiting factor to advocacy is education. O’Rourke and Outly (2024) assert that many nursing programs do not have a standardized curriculum regarding politics and processes for participation. Additionally, many professional organizations do not offer regular media training or intensive programs on advocacy and engagement. This results in varying levels of nursing education on politics or policy development for sustained activism and engagement (O’Rourke & Outly, 2024). Although significant work is required, these factors represent opportunities for targeted strategies to improve nursing engagement in healthcare policy development and evaluation.
