In order to measure and evaluate patient safety culture, the Agency for Healthcare Research and Quality (AHRQ) has developed the SOPS (Surveys on Patient Safety Culture) surveys. These surveys are designed to assess the perception of healthcare professionals regarding various aspects of patient safety culture within their organizations.

In this assignment, I will select the appropriate SOPS survey for the nursing practice setting and discuss the scoring of my facility. Furthermore, I will provide recommendations for improvement based on the survey results.

After reviewing the AHRQ SOPS Surveys webpage, I have identified the “Nursing Homes: SOPS Survey” as the most suitable survey for the practice setting in which I work. Scoring of My Facility:

Upon completion of the selected SOPS survey at my facility, several areas of strength and areas requiring improvement were identified. The survey evaluated various dimensions of patient safety culture, including communication openness, teamwork within units, feedback and communication about errors, staffing, and overall perception of patient safety. The scoring metrics for each dimension range from “strongly disagree” to “strongly agree” or “never” to “always,” providing a comprehensive assessment of the facility’s patient safety culture.

Based on the survey results, my facility scored positively in the dimensions of communication openness, teamwork within units, feedback and communication about errors, and overall perception of patient safety. These areas indicate that a strong patient safety culture exists in my facility, as healthcare providers feel comfortable speaking up about potential safety issues, engaging in effective teamwork, and receiving regular feedback regarding errors. These findings suggest that my facility has a supportive environment that fosters open communication and a commitment to patient safety.

However, the survey also highlighted areas that require improvement, specifically in the dimensions of staffing and non-punitive response to errors. The scoring in these dimensions suggests that there may be challenges in ensuring adequate staffing levels and creating an environment where healthcare providers feel safe reporting errors without fearing punitive actions. These findings indicate the need for proactive strategies to address staffing issues and to promote a just culture that emphasizes learning from errors rather than blaming individuals.

Recommendations for Improvement:

Based on the survey results and the identified areas requiring improvement, several recommendations can be made to enhance patient safety culture in my facility:
