Collaborative Practice Agreement for Advanced Practice Nurses

This Collaborative Practice Agreement (CPA) is entered into on this 7th day of June, 2018, between Hoover Family Medicine, herein referred to as “the Company,” and Maria Rodriguez-Gallo, A.R.N.P-BC, herein referred to as “the Nurse Practitioner.”

Background:
This CPA is designed to define the roles, responsibilities, and scope of practice for the Nurse Practitioner working within Hoover Family Medicine. The Nurse Practitioner, as an Advanced Practice Registered Nurse (APRN), shall collaborate with the Company’s healthcare professionals to provide comprehensive and high-quality patient care services.

Purpose:
The purpose of this agreement is to promote collaboration and ensure the delivery of safe, efficient, and effective healthcare services to patients accessing Hoover Family Medicine. The parties involved shall work together to enhance patient outcomes, promote evidence-based practice, and maintain compliance with relevant laws, regulations, and professional standards.

Terms and Conditions:
1. Authority and Responsibility
1.1 The Nurse Practitioner is authorized to provide healthcare services within their scope of practice as defined by relevant state laws and regulations.
1.2 The Nurse Practitioner agrees to collaborate with the Company and its healthcare professionals to deliver patient care that adheres to the highest standards of quality and safety.
1.3 The Nurse Practitioner accepts responsibility for their own actions and decisions while practicing within this collaborative agreement.

2. Scope of Practice
2.1 The Nurse Practitioner will utilize their advanced knowledge, skills, and competencies to assess, diagnose, prescribe medication, and provide therapeutic interventions to patients within the agreed scope of practice.
2.2 The Nurse Practitioner will refer patients to appropriate specialists or other healthcare professionals when required, and collaborate with such professionals to ensure continuity of care.
2.3 The Nurse Practitioner will keep abreast of current best practices and evidence-based guidelines in their specialty area and integrate such knowledge into patient care.

3. Practice Location and Schedule
3.1 The Nurse Practitioner shall practice exclusively within the premises of Hoover Family Medicine unless specified otherwise by the Company.
3.2 The scheduling of the Nurse Practitioner’s shifts shall be decided jointly by the Nurse Practitioner and the Company, taking into consideration the needs of the patients and efficient delivery of healthcare services.

4. Supervision and Collaboration
4.1 The Nurse Practitioner acknowledges the importance of collaboration with other healthcare professionals and agrees to participate in interdisciplinary team meetings as required.
4.2 The Nurse Practitioner will consult with a collaborating physician or healthcare professional as deemed necessary for patient care, within the boundaries defined by applicable laws and regulations.
4.3 The collaboration between the Nurse Practitioner and the collaborating physician(s) shall include regular communication, review of patient cases, and collaborative decision-making.

5. Documentation and Medical Records
5.1 The Nurse Practitioner shall maintain accurate and up-to-date medical records for all patients they serve, in compliance with applicable laws, regulations, and professional standards.
5.2 The Nurse Practitioner shall utilize the Company’s electronic medical records system for timely documentation and recordkeeping purposes.

6. Continuing Education and Professional Development
6.1 The Nurse Practitioner agrees to engage in ongoing professional development activities to maintain and enhance their clinical knowledge and skills.
6.2 The Nurse Practitioner shall attend relevant conferences, seminars, workshops, and educational programs as approved by the Company, with due consideration to the advancement of their clinical expertise.

7. Quality Improvement and Performance Evaluation
7.1 The Nurse Practitioner shall actively engage in quality improvement initiatives and performance evaluation activities implemented by the Company.
7.2 The Nurse Practitioner acknowledges the importance of providing feedback, participating in peer review, and continuous self-assessment to improve their own practice and contribute to the overall improvement of patient care within Hoover Family Medicine.
