Introduction

Ethics is an integral component of nursing practice, guiding nurses in making morally sound decisions and providing ethical care to patients. In this reflective report, I will analyze an ethical issue that I experienced using the six processes outlined by Nelson in her article, titled “Ethics in nursing education: the ethics of power and the power of ethics.” I will also discuss the major emphasis of an ethical presence in nursing practice and share an incident where I demonstrated reaching beyond the nursing obligation to have a relational practice despite the challenges faced.

Analysis of an Ethical Issue

The ethical issue I encountered during my nursing practice pertained to the allocation of limited resources in a healthcare setting. Due to budget constraints, the hospital I was working in had limited supplies and a shortage of staff. This created a challenging situation, as I had to make decisions regarding the allocation of resources while ensuring equitable and fair distribution among all patients.

Using Nelson’s six processes, namely recognition, analysis, negotiation, deciding, implementation, and evaluation, I critically reflected on this ethical issue. Firstly, recognition involved acknowledging that an ethical issue was at hand and identifying the specific dilemma. In this case, the dilemma was the limited resources and the need to allocate them appropriately. The analysis process involved examining the values and ethical principles at stake, which included justice, fairness, and patient welfare. It required understanding the consequences and potential harm that could result from inadequate resource allocation.

Negotiation was a crucial process, as it required collaborating with other healthcare professionals and stakeholders to develop an equitable and practical solution. During this stage, I actively engaged in interprofessional discussions, considering various perspectives and exploring alternative options. Deciding involved weighing the possible courses of action and choosing the one that aligned with ethical principles and met the needs of all patients. It required balancing the competing claims for resources and considering the potential impact on patient outcomes.

Implementation was the process of putting the decided plan into action. I actively participated in resource allocation activities, ensuring that the chosen course of action was followed effectively and efficiently. Lastly, evaluation involved reflecting on the outcomes of the decision and assessing its effectiveness in resolving the ethical issue. It provided an opportunity for continuous improvement and learning from the experience.

Emphasis of Ethical Presence in Nursing Practice

In nursing practice, ethical presence refers to the ability of nurses to foster an ethical environment and engage in ethical decision-making. It involves being attuned to ethical issues, dilemmas, and conflicts that arise in healthcare settings and responding to them in a morally responsible manner. Ethical presence requires nurses to uphold ethical principles, such as autonomy, beneficence, non-maleficence, and justice, while considering the individual needs and values of patients.

One major emphasis of ethical presence in nursing practice is establishing trust and therapeutic relationships with patients. Acting with integrity and demonstrating a commitment to ethical principles builds trust between the nurse and the patient, enhancing the effectiveness of care. Ethical presence also involves ensuring confidentiality and respecting the autonomy and dignity of patients. By fully engaging in ethical decision-making and providing patient-centered care, nurses can promote the well-being of patients and contribute to their overall healthcare experience.

Incident Demonstrating Relational Practice Beyond Nursing Obligations

During my nursing practice, I encountered a challenging incident where I demonstrated reaching beyond the nursing obligations to have a relational practice. I was assigned to care for a patient who was terminally ill and had limited family support. Despite the patient’s critical condition and anticipated end-of-life care requirements, I took the initiative to spend additional time with the patient, providing emotional support, and ensuring their comfort. I recognized the importance of establishing a therapeutic relationship and being present for the patient during their vulnerable moments. I went above and beyond my usual duties, engaging in compassionate conversations, and addressing the patient’s emotional and spiritual needs. This incident highlighted the significance of relational practice in nursing and how demonstrating empathy and compassionate care can profoundly impact patients’ well-being and their end-of-life experience.
