Question: What is the current evidence on the use of anticoagulation therapy in patients with atrial fibrillation and history of ischemic stroke?

Atrial fibrillation (AF) is a common cardiac arrhythmia characterized by irregular and often rapid electrical activity in the atria. It significantly increases the risk of ischemic stroke, with studies estimating the risk to be as high as five times greater compared to individuals without AF (Santangeli et al., 2018). Anticoagulation therapy, such as the use of oral anticoagulant medications, is recommended to reduce the risk of stroke in patients with AF and a history of ischemic stroke. This approach is based on the strong evidence supporting their effectiveness in preventing thromboembolic events in this population.

The current evidence on the use of anticoagulation therapy in patients with AF and a history of ischemic stroke suggests that it is highly beneficial in reducing the risk of recurrent stroke. Numerous randomized controlled trials (RCTs) and meta-analyses have demonstrated the efficacy and safety of anticoagulants in this patient population.

The landmark trials investigating the use of anticoagulants in AF patients include the ACTIVE (Atrial Fibrillation Clopidogrel Trial with Irbesartan for Prevention of Vascular Events), RE-LY (Randomized Evaluation of Long-Term Anticoagulation Therapy), ROCKET-AF (Rivaroxaban Once Daily Oral Direct Factor Xa Inhibition Compared with Vitamin K Antagonism for Prevention of Stroke and Embolism Trial in Atrial Fibrillation), and ARISTOTLE (Apixaban for Reduction in Stroke and Other Thromboembolic Events in Atrial Fibrillation) trials (Marzec et al., 2019). These trials investigated the efficacy and safety of different anticoagulants, including warfarin, dabigatran, rivaroxaban, and apixaban, respectively.

Consistently, all these trials found a significant reduction in the risk of recurrent stroke in patients with AF and a history of ischemic stroke who received anticoagulation therapy compared to those who did not (Santangeli et al., 2018). For instance, the ACTIVE trial showed that combined aspirin and clopidogrel therapy was inferior to warfarin in preventing strokes in this patient population, with a hazard ratio (HR) of 1.44 (95% confidence interval [CI] 1.13-1.84) (Kim et al., 2010). Similarly, the RE-LY trial demonstrated that dabigatran, a direct thrombin inhibitor, was noninferior to warfarin in preventing strokes, with a relative risk reduction of 34% (HR 0.66; 95% CI 0.53-0.82) (Marzec et al., 2019).

The ROCKET-AF trial compared the efficacy of rivaroxaban, a direct factor Xa inhibitor, with warfarin in patients with AF and a history of ischemic stroke or transient ischemic attack. It found that rivaroxaban was noninferior to warfarin in preventing strokes, with a HR of 0.88 (95% CI 0.74-1.03) (Patel et al., 2011). Furthermore, the ARISTOTLE trial demonstrated that apixaban, another direct factor Xa inhibitor, was superior to warfarin in reducing the risk of stroke or systemic embolism, with a HR of 0.79 (95% CI 0.66-0.95) (Granger et al., 2011). These results suggest that both direct thrombin inhibitors and direct factor Xa inhibitors are effective alternatives to warfarin in preventing stroke in patients with AF and a history of ischemic stroke.

Moreover, meta-analyses of these trials have consistently shown the superior efficacy of anticoagulation therapy in reducing the risk of stroke in this patient population. A meta-analysis including nine RCTs involving over 42,000 patients found that oral anticoagulant therapy significantly reduced the risk of stroke (odds ratio [OR] 0.61; 95% CI 0.48-0.76) and all-cause mortality (OR 0.89; 95% CI 0.81-0.97) compared to control or placebo (Santangeli et al., 2018).
